What to Expect at Your First Ultrasound Visit: Peptides & Perineural Injections

Before your first appointment in our Regenerative Medicine Clinic, we wanted to provide you with some information about what to expect.

You will first undergo an interview with one of our providers as well as our Sonologist, Dr. Michael Meng, to discuss the course of your current symptoms. Next, you will get a complete diagnostic ultrasound of your area of pain or injury, to diagnose the problem. You will then be able to choose if you would like to receive a therapeutic ultrasound-guided injection(s), intended to aid the healing of your pain permanently. If you have no findings on the ultrasound or the findings make it clear that you need surgery, you may not need the injection therapy. Most injections are intended to release any pinched nerves, break up scar tissue or speed up the repair of damaged muscles, tendons, or joints. The injection solution contains numbing anesthetic to aid in the diagnoses and to give instant pain relief. Other medicines may be included in the injection as needed. For example, if peptides or perineural injections are suggested for your treatment, you will be likely receiving a small dose of Thymosin Beta-4, BPC-157, and Vitamin B12. Thymosin Beta-4 (also known as TB-500) is the “tissue repair” peptide. BPC-157 is the “Body Protection Compound” peptide that turns down inflammation and makes the environment good for healing. Vitamin B12 (Methylcobalamin) is a vitamin that calms the nerve pain signals and gives the nerve the nutrients it needs to heal. After the treatment, if you pursued perineural injections or Hydrodissections with peptides, you will likely have the option to continue peptide therapy at home. The JWP providers will teach you how to give yourself home injections of Thymosin Beta 4 and BPC-157, and you can follow along at home with our YouTube video injection vide as well. The peptide injections will be given 5 nights out of 7, before bed, or at least 2 hours before or after eating carbohydrates or fats or drinking alcohol (injecting when you have an empty stomach will enable the best results).

More information will be given to you at the time of the treatment. If you are a person who generally doesn’t do well with needles/injections, you can choose to receive a pre-treatment dose of relaxing medicine, or you can use the ProNox machine, which is akin to ‘patient-controlled’ laughing gas that only lasts in your system as long as you are breathing it in. Some patients prefer to bring a driver on the day of their first treatment, to be able to keep track of the providers’ explanations of their diagnosis, treatment and aftercare instructions. Some patients also find that a post-treatment numbing sensation at the site of injection can make driving uncomfortable.

You will be sore for a few days after the procedure, and should not plan on any major physical activity for at least 3 days.
